Much of the efforts in sepsis care have been concentrated on acute therapy with several guidelines circulating world-wide.  However, some efforts need to be placed also on prevention as well as post-discharge morbidity and mortality issues.  These are very important in that in many instances long-term disabilities and death related to sepsis poses a greater threat especially in resource poor settings.  This presentation will address the post discharge issues and the learning objectives are as follows:

  1. An appreciation to the enormity of the burden of post discharge mortality and morbidity
  2. Identify patients who are at high risk for post-discharge morbidity and mortality will be addressed
  3. Propose interventions that may be of benefit
